This template displays the appropriate project health status icon using the given status along with the assessment criteria version used for the assessment. All parameters are optional. By default:
- project_health_status = -1 (Not Reviewed)
- criteria_version = null (Provisional)

Valid values are:
- -1: Not Reviewed (default)
- 0: Level 0 Project
- 1: Level 1 Project

- 1: Assessment Criteria v1.0
- 2: Assessment Criteria v2.0
